Dr. Crystal and Guest of Holistic Wellness Beauty  are often filming for educational and entertainment purposes. We invite you to be a guest of The Holistic Wellness Beauty Show.

Please complete the Talent Release below in acknowledgment of the terms.

*I grant Holistic Wellness Beauty  permission to use my photograph or digital recordings in conjunction with publicity, publications, marketing, and advertising on behalf of Holistic Wellness Beauty . I further consent that my name and identity may be revealed therein or by descriptive text or commentary. I acknowledge that I am giving Holistic Wellness Beauty permission to use my photograph. In addition, Holistic Wellness Beauty  agrees to be photographed by Clients and Camera Crew.

*I understand that there will be no financial or other remuneration for recording me, either for initial or subsequent transmission or playback.

*I also understand that Holistic Wellness Beauty is not responsible for any expense or liability incurred as a result of my participation in this recording.

*I represent that I am at least 18 years of age or have parental permission, have read and understand the foregoing statement, and am competent to execute this agreement.

*By entering the event, you agree that Holistic Wellness Beauty may videotape and photograph you, and record your voice, conversation and sounds. You also agree that your photo or a likeness of you may be used for promotional purposes without compensation to you.*
